Telehealth Informed Consent

By using PamoraHealth you consent to receive health-care services by telehealth from independent, licensed clinicians through affiliated professional entities.
What telehealth is
Telehealth uses electronic communication, including secure messaging, audio, and video, to deliver care when you and your clinician are not in the same place. Your clinician may rely on the information you provide, photographs, and available records. In some cases your clinician may decide that telehealth is not appropriate and refer you to in-person care.
Benefits and risks
Benefits include convenience, faster access, and continuity. Risks include the possibility that information transmitted is insufficient for good decision-making, that technology fails or is delayed, or that, in rare cases, security protocols fail. Your clinician will tell you when an in-person examination or test is needed.
Prescriptions
Medication is prescribed only when your clinician determines it is appropriate. Controlled substances are not prescribed through PamoraHealth. You may fill prescriptions at any pharmacy you choose.
Your rights
You may withhold or withdraw consent at any time without affecting your right to future care. You have the right to know the identity, credentials, and licensing state of the clinician treating you. Your records are confidential and protected by law.
Emergencies
Telehealth is not for emergencies. Call 911 for a medical emergency and 988 for a mental-health crisis.
