Terms of Service

1. Who we are

PamoraHealth, Inc. ("PamoraHealth", "we", "us") provides a technology platform that connects you with independent, licensed clinicians and coordinates related services such as scheduling, secure messaging, billing, pharmacy fulfillment, and laboratory testing. PamoraHealth does not provide medical services and is not licensed to practice medicine.

2. Eligibility

You must be at least 18 years old, physically located in a U.S. state where the relevant program is available at the time of your visit, and able to provide accurate information about yourself. You may not use the platform on behalf of someone else unless you are their legal guardian and we have approved it in writing.

3. Clinical services

Clinicians who treat you are independent professionals working through affiliated professional entities. They are solely responsible for clinical decisions, including whether to prescribe. Completing an assessment does not guarantee treatment.

4. Payments

Program fees are shown before you submit an assessment. Your payment method is authorized when you submit and charged only when a clinician approves your plan. If you are not approved, you are not charged. Medication and laboratory fees are billed separately by the pharmacy or laboratory at the price shown to you in advance. Subscriptions renew monthly until cancelled and can be cancelled at any time in the portal, effective at the end of the current billing period.

6. Your responsibilities

Give complete and truthful information, follow the instructions of your clinician, tell us about changes in your health, and use the platform only for its intended purpose. Do not share your login.

7. No emergency services

The platform is not for emergencies. If you believe you are experiencing a medical emergency, call 911. If you are in crisis, call or text 988.

8. Privacy

Our Privacy Policy and Notice of Privacy Practices explain how we collect, use, and protect your information, including protected health information.

9. Disputes

These terms are governed by the laws of the State of Delaware. Disputes will be resolved in the state or federal courts located in Delaware. Nothing in these terms limits your rights under applicable consumer-protection law.

10. Changes

We may update these terms. Material changes will be announced in the portal at least 30 days before they take effect.
